You have a couple different options to access your statements. You will have a statement if you were eligible to participate. If you are not sure you were eligible, please refer to the eligibility requirements located in the Summary Plan Description located on AGNES.

Option 2: If you prefer a hard copy and/or would like to discuss your statement and ask questions you can:
Schedule a meeting with your supervisor
Call and/or schedule an appointment with the HR Department in Slater.
